Exclusions:
Visiting Housing Support is available to people aged 16 and over, and to families, living in Edinburgh who are experiencing, or at risk of, homelessness. Referrals are accepted from anyone, including self-referrals, through the online referral link on our website. Each referral is assessed individually and fairly, without discrimination. Where we cannot offer support, we will explain why.
Assessment/referral process:
- Any agency, Court, GP, Health professional, Other, Self-referral, Social work
Referrals can be made by anyone, including the person themselves, family members, friends, justice social workers and other professionals. All referrals must be submitted through the online referral form on our website. All Visiting Housing Support providers in Edinburgh use the same referral pathway. Each referral is picked up by the most appropriate VHS team, based on locality, age and complexity of need. Once a team has accepted the referral, the referrer receives a confirmation email stating which organisation has picked it up. When a practitioner has been allocated, they notify the referrer and contact the person directly to arrange a first meeting. At the first meeting, the practitioner completes an initial assessment, including a risk assessment, covering the person’s housing situation, needs and goals. They then agree a person-centred support plan together, setting out the support to be provided and how often visits will take place. The person must agree to the referral and to receiving support.
