Details of all Specific national or local scheme/intervention(s) available

All commissioned services are noted above. Referral of individuals is based on their assessed risk and needs and referrals made by their allocated worker to appropriate support services in our area either commissioned or universal.

Possible Outcomes

All referrals are made to support the individual to address issues impacting on their social, health, welfare and behavioural needs to reduce the risk of further offending.

Process for Assessment and/or inclusion in scheme/intervention

Individuals are assessed and referred by an allocated worker in partnership with the service provider.

Where no specific scheme/intervention is available, is there capacity to provide a bespoke service based on judicial request?

Given budget restrictions, funding models and limited resources it’s highly unlikely there would be any capacity to create additional services.
