Change Grow Live – (CGL) Forth Valley Recovery Service

Aim/Purpose:

The service aims to provide high quality evidence based interventions within a recovery orientated system of care to reduce harm and initiate sustained recovery for individuals experiencing problematic substance use:

  • To reduce the harm caused by drugs and alcohol
  • To reduce the inequalities often experienced by individuals and families affected by substance use
  • To improve the health, social, psychological, legal, welfare and life chances of people who are vulnerable through the use of alcohol and drugs through minimising their substance use and promoting their recovery
  • Promote independent and healthy living
  • Offer a Trauma–informed service which take into account their history and any presenting difficulties, to offer a range of evidence based and psychologically informed interventions

Suitability criteria:

  • All genders
  • Middle-aged adult (36-55), Older adult (56 and older), Young adult (18-35)

Exclusions:

CGL work in partnership with the NHS Substance Use Service (SUS) to provide the most responsive and appropriate support and care to people. CGL and SUS meet weekly to review new presentations to the services and agree which is the most suitable service to meet people’s needs.

Given this, a person may present to CGL, but be more suitable to be supported by SUS, and vice versa. This partnership is supported by Information Sharing Agreement, NHS Forth Valley Access Policy and associated governance policy and procedures.

Assessment/referral process:

Referrals are made over the phone, through e-mail, through the Referral link on our website and for health, via Sci Gateway. When a person accesses CGL, we provide information about what support options there are appropriate to their needs, allowing them to be fully involved in decisions around what support they want to receive. If the person agrees, they will participate in an assessment of their needs in order to ensure we have a clear understanding of all relevant information and issues that are important to them. An individual recovery plan is then agreed. We talk with the person to ensure that the goals are realistic and achievable. This plan is reviewed and updated regularly. CGL use validated outcome measurement tools which allow the person to regularly self-assess progress and plan further goals.

Community based support and counselling – online, group based and individual. Active community approach to recovery. Recovery cafes and support networks/activities across Forth Valley including SMART recovery meetings.
