Aim/Purpose:

Keep well’s purpose is to increase the rate of health improvement in the population of Forth Valley, particularly with people most at risk of preventable ill health and least likely to access services early. This is achieved by providing one to one health assessments with a Nurse, using motivational counselling, brief interventions, goal setting and individual support. Health assessment and support. Improved health and wellbeing.

Assessment/referral process:

  • Self-referral, Social work

Actively targets individuals between the ages of 40 and 65, living in the most deprived areas but any individual over the age of 18 may self refer to any of our clinics within Forth Valley. Also provide outreach support to vulnerable groups such as homeless units, soup kitchens and recovery cafes. The service provides one to one health assessments for both men and women. Health assessments are available in venues across Forth Valley Criminal Justice Social Work (CJSW) Referral.
