Aim/Purpose:

Breakthrough is an innovative service provided by Blue Triangle, delivering a trauma informed, person centred approach to supporting individuals throughout their recovery journey. The Breakthrough service provides direct support and engagement in a connected community model to empower people to unlock their full potential. Breakthrough staff focus on the need of each individual and support them with their recovery journey and sustaining their tenancy, one to one support to progress towards a life free from addiction and mental health struggles.

Suitability criteria:

  • All genders
  • All ages

Assessment/referral process:

  • Any agency, Court, GP, Health professional, Self-referral, Social work

Direct referral to service provider, by phone or e mail following assessment. Individuals can self-refer or be referred by others involved in their support.
